It is a surgical procedure designed to flatten a protruding abdomen by tightening the muscles in the abdominal wall while removing excess fatty tissue and skin. Who is a candidate?

A protuberant abdomen can be the result of excess fat, weak abdominal muscles, pregnancy or loose skin that has been over-stretched and no longer responds to diet and exercise. Also, for those who have experienced a massive weight loss, their only alternative to loose overhanging skin is surgery. The procedure is very common and can benefit both men and women. Frequently, it can be combined with other surgeries such as breast augmentation, liposuction or hysterectomy. Dr. Turowski will gladly work with patient’s gynecologist to arrange for a combined hysterectomy/abdominoplasty procedure.

How is abdominoplasty performed?

The procedure is usually performed under general anesthesia to provide not only complete pain relief but also relaxation of abdominal wall necessary for optimal muscle tightening. Various surgical techniques can be used, depending on your goals and your specific problem. In some cases, a mini-abdominoplasty (combination of liposuction, muscle tightening and below the navel skin removal) may suffice. For those with severe laxity of abdominal muscles or excessive skin, a full tummy tuck may be the best solution.

During abdominoplasty an incision is made in your bikini area and after liposuction, excess amount of skin is removed and muscles are tightened with permanent sutures. In mini-abdominoplasty, no incision is made around the navel. The incision is closed right above pubic hair area resulting in low lying, transverse scar.

What is the typical recovery?

The procedure is usually performed on an outpatient basis although some patients prefer to stay overnight in the hospital. Any pain or discomfort that follows surgery can be controlled with oral medications. To avoid fluid build up the drains are placed during surgery. They remain in place for a few days until discharged fluids decrease. During your recovery you must refrain from any strenuous activity or heavy lifting for about 4 to 6 weeks. Most patients return to work after about one week.

The Mommy Makeover – Not every mom can look like supermodel Heidi Klum after having three kids and parade down the Victoria Secret runway in a tiny bikini and bra. In fact, most women don’t have such a smooth transition from pregnancy to motherhood. Many return to or start a regular workout regime to lose the extra baby weight and some go on diets to get their bodies to look like they did nine months ago. But even all of these efforts can’t help some moms get back into shape and they are surprised to see their body change so dramatically after pregnancy. Sometimes plastic surgery is the only option for mothers to revert back to their previous figure.

Many mothers turn to the “mommy makeover,” which reconstructs both the abdomen and the breasts. Depending on the severity of the changes, there are several options for women. To reconstruct the tummy, there are several different options available at my practice. After pregnancy, some mothers only find the skin on their stomach to be somewhat stretched and don’t experience much weight gain. In this case, a noninvasive treatment is usually the best—Thermage can be used to shrink the skin. This is a non-surgical skin tightening device that utilizes microwaves to contract collagen. For those who only have a little outstretched skin and small amount of extra fat, CoolLipo laser liposuction can be an excellent choice. This minimally invasive procedure will break up and eliminate fat cells, while at the same time, promote skin tightening. All this without scars. *

There are two more invasive alternatives for moms whom had more dramatic weight gain. The first is mini abdominoplasty, which is for those who have some residual fat after pregnancy and a lower abdominal skin overhang. In my practice, this is typically 10 to 20 percent of my post-pregnancy patients. During this procedure I only make a lower abdominal incision, unlike in a traditional tummy tuck, the navel would not be repositioned making women virtually scarless in the bikini. A full tummy tuck is the most popular abdominal procedure for mothers and is best for women who have gained a lot of weight and have a large amount of extra skin. This will transform the abdomen from the chest belly button down. During the two and a half hour procedure, the excess skin is removed, muscles are tightened and liposuction is performed to sculpt the stomach back into shape.

For the sake of efficiency and time, these procedures are sometimes combined with either a breast reduction or augmentation and lift. The joint procedure only takes about four to five hours. It provides the patient with many advantages: they only have to undergone one operative procedure and anesthesia, the combined operative time and recovery is shorter than doing the procedures separately, and it’s more affordable.

Unlike the abdomen, nothing can be done for the breasts without surgery. Though there is some research being done with injecting fillers such as Juvederm or fat into the breasts, neither option had been approved in the United States. Typically after pregnancy and breastfeeding, women lose volume in their breasts and they tend to sag. Many mothers require a reduction to remove the extra skin that was stretched out when the breasts were engorged with milk. Others need augmentation to morph the breasts into their shape prior to pregnancy. And some are combined with lift to rectify the sagging. An advantage of these surgeries is they can be done at any time—prior to, in between, or after pregnancy—unlike a tummy tuck, which is typically done after all pregnancies. And if breast surgery is done prior or in between pregnancies, patients usually don’t need the surgery again because extra support is provided and the breast won’t lose its shape.

Liposuction remains the most popular cosmetic procedure according to the recent statistics provided by The American Society of Plastic Surgeons. The procedure enjoys such popularity because it allows safe and effective removal of unwanted, stubborn fat deposits through very small inconspicuous incisions. Liposuction was developed in the early eighties and since then underwent many refinements including introduction of lipoplasty, liposculpture, tumescent liposuction and ultrasonic liposuction.

Who is a candidate?

Liposuction can be performed on anybody who has excessive pockets of fat and has a proper skin tone. This is not necessarily directly related to the age of patient. If the skin is loose and inelastic, some additional procedures such as abdominoplasty or face lifting may be necessary. Liposuction can result in a permanent recontouring of the almost any area of the body such as neck, chest abdomen, arms and legs. This can offer a new shape for people who never thought it was possible, giving a psychological boost and the incentive to exercise even more than before to improve the rest of their physique. However, liposuction is not a weight loss technique, and is not a cure for overall obesity. *

Is liposuction safe?

If performed by a qualified, board certified plastic surgeon, liposuction has become remarkably safe, although some risks do persists. In unusual situations, removal of very large amounts of fat can still result in moderate blood loss, and may require overnight hospital stay. Infection is extremely uncommon.

How is liposuction performed?

A very small incision is made near the area of fat to be removed. A tumescent solution (a mixture of local anesthetic and epinephrine) is first injected. This solution allows not only for pain control but also minimizes blood loss occurring during this procedure due to action of epinephrine which constricts surrounding blood vessels. Then a thin, smooth hollow tube (a cannula), with the size of a small straw, is inserted and connected to a negative pressure vacuum. The vacuum sucks the fat out leaving the skin, nerves and blood vessels intact. This allows Dr. Turowski to carefully sculpt area into a new, improved contour.

Depending on the magnitude of the procedure it can be performed under local anesthesia or intravenous sedation. In some cases, general anesthesia might be the best option.

What is the typical recovery?

To reduce swelling and bruising, at the end of the procedure, a snug-fitting garment similar to a girdle is applied over the treated areas. The procedure is usually done on the outpatient basis, so you will be able to go home the same day as your surgery. In unusual cases where the amount of fat removed is very large, Dr Turowski may recommend overnight hospital stay for postoperative monitoring.

Most patients are mobile immediately after liposuction and quiet activity can be usually resumed within a few days. However, for the best results it is prudent to wait a few weeks before resuming strenuous activity and exercises. Compression garments have to be worn 2 to 3 weeks after liposuction. You may feel bruised and sore as if you?d over-exercised that part of the body.

Does the fat come back?

Fat cells do not regenerate so the contour changes should be permanent. However, if you gain weight, the fat cells that are left behind will get bigger, just as all of the other fat cells in the body do. Still, your body should retain the improved proportions. *

What is ultrasonic liposuction?

This is the latest technique to improve and refine the results of liposuction. It utilizes ultrasonic energy or very fast vibration of the cannula to brake fibrous attachments between fat cells. It allows for more complete and smooth fat removal. Although not recommended for all liposuction procedures, ultrasonic liposuction may be very helpful in fat removal from the back, upper abdomen and especially from the male chest (ginekomastia).

Body to scalp hair transplantation has gained huge recognition in the plastic surgery hair transplantation world. Although originally implemented in the 1990s, skilled surgeons worldwide have now begun regularly performing the procedure.

Although body to scalp hair transplantation is still not able to achieve as successful or predictable results as traditional scalp hair transplants, this is a good (sometimes the only) option for patients with lower or thinner scalp hair supplies. For many years, patients with severe baldness were turned away from hair transplants and now there is finally a way to help restore hair growth even for those with severe baldness.

Thanks to Neograft FUE technology, donor hair for the scalp can now be harvested from all over the body, including the chest, shoulders, beard, stomach, arms and legs. Robotic hair transplants cannot be performed from body at this time but it can change in the near future.

This harvested body hair is usually utilized as “filler hair” along with scalp hair but in severe cases, it can be used independently. Although less predictable than traditional procedures, body to scalp hair transplants have proven to be great options, which yield successful results for many patients suffering from less viable scalp hair.

Recent FUE technological advances have truly broadened the options available for patients interested in hair transplantation, especially men. Popular new procedures now include hair restoration of the beard, goatee and sideburns. These transplants are usually done for men who have genetic predispositions to having thin or nearly absent facial hair. Men may also have suffered facial hair loss from trauma to the face, which caused scarring or previous procedures such as cleft lip surgery, or a facelift that resulted in stunted hair growth.

The donor area for facial hair transplantation is usually the back and/or the sides of the head, depending on which hair is the closest match in texture and color. A common fear of patients is how natural the results will look and smooth their facial skin will be post shaving transplanted hair. These issues are prevented when FUE grafts are properly harvested by ARTAS robot or Neograft and placed at the correct angles at recipient sites, being careful to only transplant small sections at a time.

The facial hair transplantation procedure is very similar to the process used for other hair transplants. First, follicular units ae harvested uing FUE techniques from the back or side of the head. The harvesting of FU grafts from the scalp takes a couple of hours and the patient lies face down during the hair extraction. These harvested hairs are next planted at the recipient site. Any existing hairs at the recipient site, even very thin “peach fuzz,” can be incredibly helpful in guiding transplantation and ensure grafts are placed at the correct angle of facial hair growth. The immediate post-procedure results are basically equivalent to what a beard would look like after three days of no shaving. This allows patients to instantly evaluate the success of transplantation.

The most common complaint of facial hair transplant patients is that transplanted hair sticks out and away from the face rather than remaining flat against the skin. If this occurs, it is usually only in the mustache area because protruding lips and the mobility of the skin make correctly angled hair insertion difficult at this site. If there are any transplantation problems, hair is discarded or replanted to fix any misalignments. Any prolonged redness usually disappears after about two weeks, and often times much sooner. Shaving is permitted by the eighth day post procedure. Occasionally, patients receive touch up transplants, but most are happy after just one procedure.

Breast Augmentation Chicago a Personal Story by Dr Gregory Turowski

Breast enhancement is an art. Although it is quite simple for an average plastic surgeon to place a breast implant through a large cut incision – leaving a conspicuous (sometimes ugly) scar and causing a breast bump rather than beautifully shaped breasts, this is not what women desire. Most women dream of beautifully and naturally shaped breasts without visible scars. While the size of the breasts is a uniquely personal decision, the shape, feel, symmetry and lack of visible scars is not. These details are really up to the surgeon.

This is why Dr. Turowski has been perfecting his breast augmentation procedure since 1996. From the beginning, he was looking for the most natural, scarless approach. Although most plastic surgeons discard and avoid the endoscopic armpit (axillary) approach because they find it difficult to achieve predictable results, this seems to be the preferred technique for truly experienced and skilled surgeons.

Throughout his career, Dr. Turowski has successfully performed THOUSANDS of breast augmentations combining the armpit endoscopic technique with saline implants. One reason for this is that, until last year, silicone implants still presented some challenges. While saline implants are inserted as empty bags which are rolled and filled with saline after insertion into the woman’s body, silicone implants come pre-filled and have to be squeezed through a very small incision and a tunnel from the woman’s armpit to her breast pocket. Therefore, Dr. Turowski’s preferred incision for silicone implants used to be periareolar (still better than inframmary – under the breast scar).

This issue finally improved in 2012 when a special funnel (Keller funnel) was created to expedite touch-less silicone implant placement. Through this ingenious method, silicone implants can now be placed effortlessly through a minimal, usually invisible armpit incision, which creates beautiful and natural looking breasts without ANY breast scars. Moreover, the Keller funnel avoids any contact between an implant and the armpit skin, which could potentially lower the incidence of capsular contractures.

As a result, Dr. Turowski now performs most of these surgeries (silicone and saline) through a minimally invasive armpit incision (although other techniques are obviously available upon request). Typically, women are back to their daily activities, with minimal swelling and bruising, within 48 hours of surgery.

Another recent development in breast implants is an addition of the Sientra and Inamed shaped form stable implants . These cohesive “gummy bear” implants might be helpful in some cases and Dr. Turowski discusses all of these options during personal consultations.

First introduced in 1962, since 1992 until 2007, silicone breast implants had been available for research purposes only ( Dr. Turowski was a part of this research effort). However, the last chapter of the silicone breast implants saga was closed in November 2007, when (after 14 years of deliberations and research) the FDA decided that the medical evidence was sufficiently positive enough to let these devices back on the market. The studies have failed to find a link between silicone implants and cancer, connective tissue disorders or other diseases.Science has prevailed. US women now have the same options as others around the world had for years.
Breasts augmentations have grown in popularity despite media controversy. Last year over 300,000 women had their breast enlarged in the United States, a 40% increase since 2000. Until 2007, most procedures utilized the saline or salt water solution, rather than silicone, implant. However, since silicone breast implants have been officially re-approved for primary breast augmentations, this situation has completely changed. Currently, in Dr. Turowski’s practice, 9 out 10 women choose silicone implants over saline implants. This follows the worldwide trends, which show that over 90% of breast enlargement procedures utilize silicone rather than saline implants. The advantage is clear: silicone feels softer and looks more natural.

Now some regulatory issues: the FDA is allowing silicone devices for women who are at least 22. The only exception is for women with inborn anomalies such as asymmetry, tubular breasts and some other conditions (if you are not sure if you are a candidate for silicone implants, please schedule a consultation with one of our surgeons to discuss your situation). Saline implants are still widely used, and for some patients, maybe a more desirable choice. Saline implants are still associated with a very high satisfaction rate.

Implants are not considered lifetime devices and some patients will need at least one additional surgery to remove and replace implants. MRI studies may be recommended post surgeries. Manufacturers of implants offer life time warranty on these devices and defray costs of replacements for 10 years.

How is breast enlargement done?

The procedure consists of creating a pocket under the breast tissue and pectoralis muscle and placing a soft, natural-feeling implant underneath the breast and the pectoral muscle. Our surgeons perform all augmentations in the submuscular position. In rare cases, the exact position of the implants (subglandular or submuscular) can only be recommended by the plastic surgeon after the clinical evaluation of the patient. There are a variety of implant designs and your plastic surgeon will review the potential advantages of certain designs during a consultation.

What kind of scars can I expect?

Since the saline implants are inserted deflated and are later injected with saline when inside the body, the incision can be kept to a minimal size (usually shorter than an inch). Moreover, although incisions are small, every attempt is made to hide them either in the arm pit or under the areola, where they can be barely noticeable on the border of lighter and darker skin. The axillary (armpit) approach requires minimally invasive endoscopic video techniques which are readily available to our plastic surgeons. Silicone implants are fully inflated before insertion and therefore the incision has to be slightly larger. However, scars resulting from these surgeries are still usually barely noticebable if made around the areola or in the armpit. We usually try to stay away from the under the breast (inframmary) insertion sites since the healing in this area is quite unpredictable and may lead to conspicous scarring. Since its introduction, the Keller funnel armpit approach is the best technique to hide scars from both saline and silicone implants.

Am I going to loose nipple sensation?

Minimally invasive techniques utilized by our plastic surgeons very infrequently result in nipple sensation alteration. Nipple sensation loss is extremely uncommon and is independent of the incision site chosen. The nipples may actually be hypersensitive (e.g. more sensitive) for some time after surgery. Under the breast incision has been shown to cause most nipple sensation loss (one more reason to avoid it)

What kind of anesthesia is used for breast augmentation?

With current advanced anesthesia techniques, the best and safest way to perform breast augmentation is to do the procedures under deep intravenous sedation with patients totally unaware of the ongoing surgical action. This allows for pain-free surgical procedure while permitting avoidance of risky chest wall injections for local pain control. Postoperative nausea is minimized using this approach and there are no complaints of postoperative sore throats and hoarseness, which is usually typical for general anesthesia and tracheal intubations. We also routinely utilize OnQ pain pumps which inject local anesthetic directly into the implant pocket during the first several hours after sugery.

How long does the procedure take?

Although the surgical time for breast augmentation may vary depending on a particular patient’s situation, surgery generally requires less than one hour of anesthesia time.

Do I have to stay in the hospital?

Breast augmentation is typically an outpatient procedure (patient returns home after she is recovered from anesthesia) and we perform it in our AAAHC accredited surgery suite. However, to accommodate some patients’ social needs, an overnight stay in a hotel and nursing support can be arranged.

What is the typical recovery?

After surgery, the patient is wrapped in a compressive dressing. The following morning the dressing is removed and she is placed in a soft bra. The discomfort after breast augmentation can be significant. Therefore, our plastic surgeons use the revolutionary On-Q pain control system “pain pump.” This disposable device, which is placed during surgery, delivers local anesthetic directly to the operative site, providing over 24 hrs of pain relief. The device is then easily removed during the postoperative visit day after surgery . Routine activities can usually be resumed within a few days. Sports and exercises can be resumed within a few weeks.
